Use entered or dictated observations as the source. Where enabled, drafting and translation assistance can prepare wording for staff to check, helping them revise the note without rebuilding every sentence.
Your people verify facts, correct AI drafts, sign records and approve consequential actions.

The work behind the review
The staff member knows what happened. Writing it clearly is another task.
Wording, transcription or a language difference can add another round of rewriting at the end of a shift. Assistance is useful when it gives the author a clear starting point while keeping their observations and responsibility intact.
01 · A specific review step
Capture what the author actually observed.
Start with the activity, support provided and observable response. Record the required person, service and timing context through the appropriate workflow.
If an important fact is missing, ask the person who knows what happened. A polished draft should not add an unobserved assistance level, mood or outcome.
- Keep direct observations distinct from later explanations.
- Do not treat professional-sounding wording as a new fact.
02 · A specific review step
Use available assistance as a draft.
Where enabled for the agency, LunahUP supports note-preparation assistance such as wording, transcription or translation. Confirm the specific tool, supported input and permissions during your walkthrough.
Existing synthetic proof showed proposed wording, flagged missing information and required staff confirmation. That bounded result does not establish that every transcription or translation is accurate, or that a complete signing path was demonstrated.
- Compare the proposed wording with the supplied source.
- Correct or reject a suggestion that changes the meaning.
03 · A specific review step
Check the details that can change the record.
A fluent translation still needs a meaning check. Verify who did the activity, what support staff provided, the person’s response, and relevant numbers or timing.
For a fictional example, staff facts might be: Sam prepared a sandwich; staff gave one verbal reminder to wash hands; Sam washed hands and finished. A draft can organize those facts. It cannot add independence, progress or a service duration that was not supplied.
- Clarify an uncertain phrase with its author.
- Use the agency’s appropriate process for reviewing language and accuracy.
04 · A specific review step
Finish through the authorized human workflow.
Staff remain responsible for the source facts, corrections and signature. Supervisors apply the agency’s review requirements. AI does not sign, establish that a service happened or decide billability.
During the agreed trial, compare the same note task: preparation, rewriting, clarification and reviewer effort. A useful result is fewer repeated steps with a review your people can still complete accurately.
- Include incomplete facts and an incorrect suggestion in the demo.
- Confirm that staff can correct or reject prepared wording.
Questions before the next step
Know what the workflow establishes.
Is an AI translation guaranteed to preserve every fact?
No. Staff must check meaning and details against the source and resolve uncertainty before accepting the draft.
Will AI sign the note for the author?
No. The responsible staff member completes the authorized human signing process.
Does the reviewed example prove all AI documentation tools?
No. It establishes bounded note preparation and staff confirmation. Other tools and supported inputs need their own availability and output review.

Use fictional observations to inspect the draft, an important missing fact and the check that stays with the author.
