A practical checklist for IDD & HCBS agencies
Before you switch, follow one record all the way through.
Choose the recurring record problem behind the extra calls and rewriting. Use these six checks to see what your staff would enter, what a reviewer must verify and what still needs agreement before a trial.
People verify the facts, review AI drafts and approve consequential actions.

The work behind the review
A feature list cannot answer the next reviewer’s question.
Walk through the task your team actually repeats. Start with sample facts, include something incomplete and inspect the result. This checklist organizes a software evaluation; it is not an official survey instrument or a finding that your agency is ready.
01 · Name the repeated work
Choose one task with a clear start and finish.
Use a recurring care note, missing-punch investigation or evidence request. Define what starts the work and what the responsible reviewer needs at the end.
- Who does the work, and who reviews it?
- Which searches, repeated entries and follow-up conversations happen today?
- What result would make the next review easier?
02 · Follow the source
Keep the facts within reach of the reviewer.
Inspect the person, date, source record and supporting document in the sample. Test a missing or unavailable source as well as a complete example.
- Can the reviewer open the source they need?
- How does the team distinguish missing facts from unclear wording?
- Where can the next person find the reason for a correction?
03 · Test the human decision
Check what happens when the draft is wrong.
Supply an incomplete sample. Where AI assistance is enabled, examine the proposed wording, identify anything unsupported and check how staff correct or reject it.
- Who provides the facts and checks the draft?
- Which actions require review or signature?
- Does the recorded result show what your reviewer needs?
04 · Confirm program fit
Check the requirement behind the feature.
Bring your service, current program requirements and required connection. Confirm the available configuration with the people accountable for the work.
- Which required forms and fields are available?
- What is supported now, what needs configuration and what is outside scope?
- If a connection is required, what response establishes receipt or acceptance?
05 · Agree access and continuity
Plan the records before introducing agency data.
Use sample information until the appropriate people have reviewed access, data handling and trial scope. Include historical records and unfinished work in transition planning.
- Which roles can see and change each record?
- Who checks any imported information against its source?
- What remains available during and after the transition?
06 · Define the evaluation
Compare the same task, including the review.
Record the current process and compare it with the agreed available workflow. Include preparation, correction, searching and reviewer effort in the comparison.
- What will your team observe during the 30-day free evaluation?
- Which unresolved items must be settled before a rollout?
- Who decides whether the workflow fits?

Questions before the next step
Know what this guide establishes.
Does checking every box mean we are compliant?
No. These are personal planning notes for a software evaluation, not verified findings, official requirements or a compliance score.
Will the checklist be sent to LunahUP?
This checklist keeps the checks only on the current page. It sends nothing and does not save them after a reload.
Does the evaluation include a complete migration?
The 30-day free evaluation covers an agreed scope. Migration formats, validation responsibilities and costs are separately agreed.
